What Causes Central Serous Retinopathy?

Central Serous Retinopathy (CSR) is a condition in which fluid accumulates under the macula only, causing it to lift and resulting in visual disturbances. The macula is a vital part of the retina needed for sharp, detailed sight.

The leakage in CSR is due to the excessive leakiness of large blood vessels beneath the retina.

What Are Some Symptoms of CSR?

Blurry vision, like a smudge in the center of your sight.
Dark spot in the center of your vision.
Darker or dim vision.
White items look dull or somewhat brown.
Objects seem smaller or further away than they are.
Straight lines look crooked or bent.

However, CSR doesn’t always cause symptoms. A person can have the condition but not have apparent visual problems.

How is CSR Diagnosed?

CSR is diagnosed through the following tests:

Visual acuity test: This test will be done to assess the sharpness of your vision.
Optical coherence tomography (OCT): An OCT scan of your macula will be performed to measure retinal thickness, identify swelling and detect serous retinal detachment.
Fluorescein angiography (IVFA): This test identifies the location of the leak and assists in directing treatment. For IVFA, a healthcare professional will inject a dye into a vein in your arm. The dye spreads through your body and into your retina. Then, pictures of your retina are taken to identify leaks.

How is CSR Treated?

Although most cases of CSR resolve spontaneously after some time, some select groups of CSR require treatment. Options include:

Medications: Some medications may help treat central serous retinopathy. Oral medications, such as eplerenone, may be suitable in certain cases. For example, in specific subtypes of CSR, anti-vascular endothelial growth factor can treat new abnormal blood vessels in your eyes.
Photodynamic therapy (PDT): This combines verteporfin medication with a laser to reduce fluid buildup and is an essential treatment for those with recurrent or persistent chronic cases, or cases with multiple leaks.
